How It Works

If you or your loved one does not have MO HealthNet, you will need to apply for it.

Even if a person has been denied before, HCB (Home and Community Based Services) services have special income
and resource limits that may help them qualify.
If you or your loved one already has MO HealthNet, you do NOT need to submit another
application, simply call 877-304-7939 to ask for a referral.

Eligibility depends on your income, age, health, and individual needs.

Complete Form

You must complete & submit the Supplemental Form (Forma Español) with your application if you:

Are age 65 or older
Are blind or disabled
Get Social Security
Live in a medical or nursing facility
Have Medicare or VA healthcare

Once you've completed the application process and it is accepted, it's time to start planning your care. Care plans aid in your recovery and healing after a hospital stay by supporting you in the comfort and convenience of your home. Personal Care

Helping patients maintain their personal health and hygiene (duties include but not limited to bathing and dresssing).

Assistance with running errands, light housekeeping duties such as doing laundry and dishes to ensure patients' home healthcare health and well-being.

Meal prep includes the careful preparation of your daily meals.  Meal prep assistance slices, dices, mixes, cooks and packages food on your behalf so you can focus on your health and enjoying life. 

Nursing services (like medication set-ups, blood draws & diabetic nail care)
Respite care to help offer short-term
relief for caregivers.
